In connection with my application for employment (including contract for services) with you, I understand that an investigative consumer report is being requested from DAC Services, Tulsa, Oklahoma, that will include information as to my character, work habits, performance, and experience, along with reasons for termination of past employment from previous employers. Further, I understand that you will be requesting information concerning my driving record and/or information from various federal, state, and other agencies which maintain record concerning traffic offenses, accidents, etc., as well and information from DAC concerning (1) previous driving record requests made by others from such state agencies; (2) state provided driving records; (3) claims including me in the files of insurance companies.
I AUTHORIZE, WITHOUT RESERVATION ANY PARTY OR AGENCY CONTACTED BY DAC TO FURNISH THE ABOVE-MENTIONED INFORMATION.
I have the right to make a written request within a reasonable period of time to receive additional detailed information about the nature and scope of the investigation. I hereby consent to your obtaining the above information form DAC, and agree that such information which DAC has or obtains, and my employment history with you if I and hires, will be supplied by DAC to other companies which subscribe to DAC's services.

I understand that I have the right to review information provided by previous employers, have errors corrected by previous employer and resubmitted to Impact and/or have a rebuttal statement attached to errorneous information if my previous employer and I cannot agree on the accuracy of the information. I understand that I must request past employer information obtained by Impact in writing within 30 days of my application.
I hereby authorize former employers to release to Smithway Motor Xpress, Inc. any positive controlled substance tet results; alcohol test of .04 or greater; evidence of refusals to be tested; and information on any required substance abuse porfessional evaluation, determination of driver's need for assistance, and driver's compliance with these recommendations for the three years preceding today's date. I request that such records be released immediately
